St. Joseph's Health uses Oracle Health AI to close care gaps and sustain lower readmission rates

St. Joseph's Health, a New Jersey-based health system, implemented Oracle Health Quality Management, Oracle Health Care Coordination Intelligence, and Oracle Health AI Data Platform to unify patient records that had been scattered across disparate EHR and payer systems. AI capabilities embedded in the Oracle Health solutions, including AI-powered agents for clinical documentation, provide physicians with relevant data in context and help clinicians identify and close care gaps earlier. Using advanced data analytics to identify patients repeatedly readmitted for congestive heart failure, St. Joseph's Health implemented a targeted care program that produced a sustained reduction in readmissions, keeping its readmission rate below the market average for three consecutive years.
